HOW TO SEND A HAIR SAMPLE FOR ANALYSIS

1. If this is your first time testing with us, print and fill out the three-page New Client Form. If this is a repeat hair test, print and fill out the Retest Form.

2. You can either come in and we can cut your hair sample for you, or you can read the Hair Sampling Instructions and cut it yourself. It’s easy to cut the sample yourself, but be sure to follow the instructions carefully. To set up an appointment for us to cut your hair sample for no additional fee, or if you are having trouble and need help cutting it yourself, call (818) 745-9558.

3. Mail your forms and hair sample along with your payment to 815 W. Kenneth Rd., Glendale, CA 91202, USA. To pay by check or money order, make the payment to Moses Nutrition. Be sure to make your payment in U.S. Dollars.

We accept VISA, Mastercard, AMEX or Discover. To pay by credit card, please fill out and include the credit card section of the New Client Form or Retest Form above, or fill out and include this Credit Card Info Form.
